关于开展2017年海门市专职责任督学网络问卷考核的通知

各中小学校：

我市于2013年启动全市责任督学挂牌督导工作，2015年底开始创建“全国中小学校责任督学挂牌督导创新县（市、区）”。2016年底，我市通过了省级验收，今年我市将迎接国家级验收。今年上半年，海门市人民政府教育督导室将全市中小学、幼儿园划分为18个责任区，并聘任了29名专职责任督学（中小学27名）。现根据国务院督导委员会颁发的《中小学校责任督学挂牌督导办法》第十条：教育督导部门要建立责任督学考核制度。对全市中小学校的责任督学履行职责、开展工作和完成任务情况进行考核。现经海门市教育局、海门市人民政府教育督导室研究决定，本年度将采取网络问卷的方式，对全体中小学的责任督学进行考核，请各校积极支持和配合，并对责任督学挂牌督导工作提出宝贵的意见。谢谢！

网络问卷考核说明：以责任区为单位进行考核，每个责任区一个二维码。请各校组织全体教师通过手机扫码对本校责任督学进行问卷考核，截止时间：2018年1月4日晚前。二维码见附件照片。
